Case Management
services help to improve consumer functioning in the least restrictive
environment and increase consumer involvement in the community.
Services are provided based upon an ongoing assessment of needs
at varying levels of intensity. A variety of services are provided
to help monitor, stabilize and maintain the consumer in the community.

Emergency
Services |
Emergency
services assists adults experiencing psychiatric crises by helping
to de-escalate the situation through crisis counseling, mobilizing
the consumer’s natural support systems such as family members,
and/or referral and linkage to community resources and services.
Telephone, office
and/or mobile outreach crisis services are provided 24 hours a day,
7 days a week.

Medical
Services |
Services
are provided by either Licensed Practical Nurses, a Registered Nurse
or by a dietician and include TB testing and reading, coordination
of prescription and laboratory services, dietary consultation and
occupational health services to CCC employees. Medical services
helps consumers manage their psychiatric or medical illnesses by
providing individual or group education about their medications
or mental health illnesses.

Outpatient
Services |
The Outpatient
program provides therapy to adults, children and adolescents with
a wide variety of mental health needs. We use a brief, strategic
and solution based therapy to decrease symptoms of distress, to
help return people to a higher level of functioning and to improve
the quality of our consumer’s lives. The emphasis is on group
therapy. Groups address some of life's
most stressful issues.
Outpatient therapists
have a master’s degree and are licensed or license eligible
in the State of Illinois. They actively refer consumers to additional
professional services at CCC or in the community, including support
groups.
Outpatient counselors
have a master’s or bachelor’s social services degree.
They work with children whose issues may be best addressed in the
community. Consumers may be seen at school or in other community based
sites. Individual, group and family sessions are used to work with
children and their families.

Psychiatric Services
|
Services are
provided by a staff psychiatrist to consumers during regular week
day hours. Consumers participate in an initial evaluation, which
is reviewed on an annual basis. Consumers are assessed for medication
needs including the administration of medications, medication education
and medication monitoring. Medical Services staff work with the
psychiatrist in the referral to and follow-up of medical laboratory
work as directed by the psychiatrist.

Psychosocial
Rehabilitation (PSR) |
The Pathways
program is a psychosocial rehabilitation program providing individual
and group services to seriously and chronically mentally ill consumers.
The Pathways program focuses on long-term recovery, helping consumers
to become more self-sufficient and independent.
Supported Employment
services are also provided to consumers. Staff provide on-the-job
training and support to help consumers develop and maintain work
skills and manage changes in the work environment.

Residential
Services |
Residential
services to chronically mentally ill males are provided through
three group homes. The group homes provide highly structured, intensive
treatment in an environment less restrictive than a hospital setting.
Services include psychiatric treatment, medication education and
compliance training, counseling services, independent living and
other life skills.
The Main Street
Group Home is a 24-hour supervised home setting for eight adult
males with histories of frequent psychiatric hospitalization.
Holland
Avenue Group Home is a 24-hour supervised setting for six adult
males with histories of frequent hospitalizations and challenging
behaviors that have prevented or interfered with placement in other
community settings. Two respite beds for consumers recently discharged
from Alton Mental Health Center that have not finalized stable housing
are also provided at Holland Avenue.
The State Street
Group Home offers a 24-hour supervised setting for six adult females
with serious mental illness. State Street opened in October 2003.

Screening,
Assesment and Support Services (SASS) |
SASS is a program
for youth in Madison County 17 and under who are at risk of psychiatric
hospitalization or loss of placement due to their mental health
issues.

CARES
hotline at (800) 345-9049
CARES workers
will ask questions to determine the severity of the child's circumstances.
If they believe a screening is necessary, CARES will then contact
CCC to provide crisis assessment services.
SASS also utilizes
other services in the community to prevent the hospitalization of
the youth.

Senior
Services |
Comprehensive
assessment, consultation and treatment services are provided to
residents of Madison County who are 60 years of age or older. Services
are provided in cooperation with the Illinois Department of Aging,
Southwestern Illinois Visiting Nurse Association and Chestnut Health
Systems. Referrals of seniors may come from the consumer, family
members, senior centers, physicians, or hospitals.
Services including
support and counseling with the consumer or their family, crisis
intervention and referral and linkage to other community supports
are provided to help the senior consumer continue to live in the
community in the least restrictive environment.

Substance
Abuse Services |
Substance abuse
services available at CCC include Methadone Maintenance Treatment
(MMT) and drug-free treatment. The MMT program is a risk-education
model that helps to decrease the high risk behaviors of substance
users. Daily dosing of Methadone is available for the consumer in
the MMT program. A physician supervises the treatment and medical
needs of MMT consumers.
Individual and
group services are available to all consumers in substance abuse
services. Relapse prevention, coping skills and recovery issues
are some of the topics addressed in these services.
Services to
Federal Aftercare consumers are provided in both the Substance Abuse
and Outpatient programs. Individual, family or group therapy is
provided to Federal parolees to aid in their re-integration to the
community and prevent further legal difficulties.

Costs
for Services |
Fee assessments
are necessary to determine how the costs of providing services to
the community will be supported. Consumers are asked to bring to
their first appointment proof of income (check stubs, tax returns,
SSI award letters etc) and any Medicaid, Medicare or insurance information.
A short interview with a consumer account representative will take
place to discuss fees for services. A sliding fee scale is available
when no third party coverage exists. No one is denied service based
on an inability to pay.
Consumer Fees
- A discounted
fee is offered for most services to those consumers without Medicaid,
Medicare or private insurance. The sliding scale is a form of
financial assistance to those who are unable to pay the full cost
of service.
- Once eligibility
for financial assistance is identified, proof of income and family
size is required so that fees may be assessed appropriately using
the sliding fee scale.
Illinois
Medicaid
- Upon proof
of eligibility, Illinois Medicaid is billed the full cost for
covered services.
- If services
are paid by Medicaid, the consumer is not subject to any out-of-pocket
costs.
- In the event
Medicaid eligibility ceases, the consumer will be assessed a fee
based on the sliding scale.
Medicare
Part B
- Upon receipt
of a signed assignment of benefits and copy of the Medicare card,
Medicare is billed the full cost for covered services.
- Consumers
are required to pay the appropriate Medicare co-pays and deductibles.
Private
Insurance
- With permission
from the consumer, coverage is verified by contacting the insurance
company.
- Upon receipt
of a signed assignment of benefits and copy of the insurance card,
the insurance is billed the full cost for covered services.
- Consumers
are billed appropriate co-pays and deductibles after the insurance
is billed and responds. An estimate, when available, of the out-of-pocket
cost will be given to the consumer at their first appointment.
This portion is subject to change upon determination by the insurance
company of amounts payable.
